I usually fit a socket on the end of the extractor and use a ratchet for
extra leverage.  But like Mr. Dude said, get a deep hole to get it in there
good, and use the largest extractor you can fit.

Using too small an extractor on a really stuck bolt will usually break the
extractor, which starts you on a whole new world of headaches.
