There are two retaning clips that hold the wheel bearing in place. It might
be possible to over tighten the axle nut squashing the inner bearing race
between the hub and axle surfaces they mate too, its highly unlikely.
But, that rear retaining clip could have been damaged or moved like Wayne
said, causing the entire hub to sit further back into the knuckle than
normal, which would cause the rotor to be off center. Ill bet thats what
happened.
