Take a close look at the broken area of each tooth on the hub.  If there is a
shiny spot that looks like it was polished, that tooth had a crack which
finally fatigued to the point of failure.  Once one tooth lets go, the rest
will break because they can't handle the additional load.  A crack could have
been present in the original blank.  If there are no shiny spots, then the
failure was purely strength related.
