Help needed - CV failure?

My CV/axle felt "tight" after it started made a huge vibration during
qualifying earlier this year.  I thought the wheel was going to fall off,
but when I got back to the pits, the car drove fine, and everything checked
out OK.  During the race, the CV/axle officially blew up. :)

Joe Henderson had the same problem last year during a NASA race with the
Rosen Autosport Rental.  Vibration, pull off, everything checks out, drives
fine, blows up later during a race.
