Hard starting

Suddenly the '92 is having difficulty starting on the first start of the
day.  I would call it a cold start problem but around here "cold" is
relative, it is in the 90's.  Battery good (and starter spins properly) and
changed plugs, rotor, cap but not wires yet in an effort to pin it down.
Fuel filter less than 5K miles FWIW.  Car has always lit off immediately
with no throttle input.  Now, even with some throttle, it cranks and cranks
for a bit (like 3-5 seconds which seems like forever as you know) before
starting and then does not really want to run initially but settles down
fairly rapidly.  Warmed up steady state idle seems a little rough but might
just be SOTP worrying.  Today after being fully warmed up it died twice in
traffic after large throttle inputs followed by no throttle.  Guesses?
Plugged EGR/EGR line?  Dirty MAF?  That fuel filter?  Fuel pressure?  Any
help appreciated.
